First Visit

Our dentist and team want you to be prepared for your first visit if you are a new patient here at Holladay Dental Excellence. We invite you to take a look at our patient forms and to fill them out prior to coming in. We also encourage you to bring:

• A form of ID, such as your driver’s license
• A dental insurance card, if applicable
• Any pertinent medical information or recent dental X-rays
• Your beautiful smile

Your visit will start with a brief office tour and a chance to meet our wonderful team members. Then Dr. Val Ludlow will do a thorough dental exam to check for cavities, gum disease, oral cancer and any other dental concerns that may be found. If we detect any, we can discuss recommended treatments, as well as your own personal dental goals. If there is enough time, we will also perform a dental cleaning. If any further treatments need to be scheduled, we can do so at the end of your first visit with us.
